KRS 338.031(1)(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a.On or before September 10, 1998, the employer did not provide training to ensure the original and continuing competence of personnel caring for, setting up, inspecting, and maintaining of horizontal injection molding machines, and the employer did not instruct personnel on the safe method of work prior to performing any work on the Van Dorn Number 52 injection molding machine located in the Production Molding Department.
