Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees; and a. The basket used to lift personnel on the forks of the Toyota forklift did not conform to ANSI B56.1 for the design and adequate precautions were not taken to ensure the safety of the employee being lifted. b. The fork extensions for the forklifts in the plant were not designed in accordance with ANSI B56.1. The extensions had a length greater than 150of the length of the forks on the forklift.s
