were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of flying debris:
At this location, in the Mechanic Shop, employees were exposed to a
struck-by
hazard in that 100 psi. compressed air was piped through the area via
3/4-inch,
schedule 40 Poly-Vinyl-Chloride (PVC) piping.  Plastic pipe and its
connections
are subject to failure under the stresses posed by compressed gases.  This
failure
could result in a rupture of the material, releasing shards of plastic
capable of
causing serious physical harm from punctures and/or sections of this
unthreaded
piping (held together with adhesive) could be propelled across the area
seriously
injuring anyone in its path.  PVC piping is approved only for liquids under
pressure and is unsafe for the transmission of any gas, even at low
pressures.
A means of abatement would be to replace the existing PVC piping with a
type
suitable for the transmission of compressed gasses.
