29 CFR 1926.302(b)(2): Safety clips or retainers were not securely installed or maintained on pneumatic impact (percussion) tools to prevent attachments from being accidentally expelled: Employees were exposed to the hazard of being struck by the metal connections located on the end of the airline hoses which were attached to the moisture collector/trap, while employees were accessing/egressing the scaffold ladder in the second (2nd) heater/furnace, located in the Olefins II Unit. IN THE ALTERNATIVE: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard: Employees were exposed to the hazard of being struck by the metal connections located on the end of the airline hoses which were attached to the moisture collector/trap, while employees were accessing/egressing the scaffold ladder in the second (2nd) heater/furnace, located in the Olefins II Unit. Among other methods, one feasible and acceptable method for abatement is to properly secure the hose couplings with safety clips, retainers or other positive means to prevent couplings from becoming accidentally disconnected in accordance with manufacturer recommendations.
