Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard:
Employees were exposed to a fall hazard of approximately fifty-eight (58)
feet to
sixty-eight (68) feet to ground level due to improper usage and placement
of the
Crosby clips.
a.The employer had used two (2) Crosby clips on a each end of a one-half
(1/2) inch wire rope lifeline where it passed through a drilled hole in the
steel flat bar at the top of an upright steel pipe support stanchion.  The
dead end was secured alongside of the live end of the one-half (1/2) inch
wire rope.
b.The employer had improperly placed the Crosby clips on each end of a
one-half (1/2) inch wire lifeline where it passed through a drilled hole in
the steel flat bar at the top of an upright steel pipe support stanchion.
The
dead end was secured alongside of the live end of the one-half (1/2) inch
wire rope, i.e. The saddle of the Crosby clip had been installed on the
dead end of the wire rope and the "U" bolt side of the Crosby clip had
been installed on the live end of the wire rope thus crimping the wires of
the wire rope and impairing their integrity.
c.The employer did not provide a properly formed eye termination at both
ends of the one-half (1/2) inch wire rope horizontal lifeline; at the two
(2)
ends where the turnbuckle was connected.  A properly formed eye
termination can be formed by incorporating a properly sized thimble.
This is where the one-half (1/2) inch wire rope horizontal lifeline passes
through a drilled hole in the steel flat bar at the top of an upright steel
pipe support stanchion on both ends and in the center where the one-half
(1/2) inch wire rope passes through the eye bolt on each end of the
turnbuckle.
This occurred on or about January 26, 2000 at the new U.S. Federal
Courthouse
construction jobsite at 1200 North Water Street, Corpus Christi, Texas
78401.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be installation of a properly installed one-half (1/2)
inch wire
rope lifeline using three (3) Crosby clips properly placed and with the
saddles of
the Crosby clip placed on the live side of the one-half (1/2) inch wire
rope
horizontal lifeline, along with the use of a properly formed eye
termination which
is formed by incorporating a properly sized thimble, as required by the
manufacturers recommended specifications and as specified in ANSI Z359.1
