Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fire or explosion hazard:
Employee was exposed to the hazard of fire and explosion due to the
improper
rigging of the Geronimo Emergency Escape and installation of the Geronimo
Emergency Escape Device on the Geronimo Emergency Escape Line due to:
a.The Geronimo Emergency Escape Device and Line were installed next to
the monkey board 65 feet above the ground with the line running down
the west derrick leg with no safe egress.
b.The Emergency Escape Line was not anchored/staked out as
recommended by the manufacturer Geronimo Manufacturing, Inc.
This occurred on July 10,2000 on Corpus Christi Drilling & Workover L.L.C.
Rig #5. Among other methods for abatement, one feasible and acceptable
abatement method to correct this hazard would be installation of a
properly
rigged
and secured emergency descent device or other satisfactory auxiliary means
of
egress, such as specified in Section 5, Sub-Section 5.7, Paragraph 5.7.1 of
American Petroleum Institute (API) RP 54-92, Page 12. Essential elements
of this
procedure would include:
1.Staking out securely anchored one-half inch wire rope line to the derrick
or mast so as to provide a ready and convenient means of escape from the
derrick mans's working platform.
2.The escape line route is to be kept clear of obstructions.
3.A safety device is to be provided to permit fast emergency exit from the
derrick man's working platform to a safe place.
4.The tension on the escape line is to be periodically checked and adjusted
to assure a safe landing and to allow the derrick man to stop 20 to 25 feet
from the anchor point.
5.The ground anchor point of the escape line is to be located a minimum
lateral distance from the derrick or mast equal to the height above ground
level to the point where the escape line is fastened.

were exposed to a collapsed mast (derrick) hazard:
The hazard of collapsed mast (derrick) of a drilling rig resulting from
failure to
install wire rope guylines with correctly installed Crosby clips on all
four side of
the rig as recommended by the manufacturer's specifications for the safe
guying
of the mast to ensure adequate anchorage in the following:
a.Four guylines incorrectly installed Crosby clips.
b.Geronimo emergency Escape Line incorrectly installed Crosby clips.
c.Catline on the rig floor incorrectly installed Crosby clips.
This occurred on July 12, 2000 on Corpus Christi Drilling & Workover L.L.C.
Rig #5. Among other methods of abatement, one feasible and acceptable
abatement method to correct this hazard would be installation of a properly
secured guylines as specified in the American Petroleum Institute Second
edition,
paragraph 3.14 of Spec 4e and American Petroleum Institute Standard 4d
sixth
edition paragraph 3.4 and American Petroleum Standard 4f, appendix c16,
first
edition May 01,1985.Essential elements of this procedures would include:
1.Hand screwed in auger anchors into the soil according to the
Manufacturer's recommended specifications.
2.Wire rope guylines properly secured to the raised portion of the auger
anchor.
3.Also correctly install and properly secure the Crosby clips to the ends
of
the wire rope guylines.
