a.  Marriott Hotel, Ohio & Michigan Avenue - On or about July 8, 1998, an
employing
installing insulation from an aerial lift, #2678, was not wearing a safety
harness to prevent
him from falling approximately 25 feet to the ground.
A feasible and useful means of abatement includes, but is not limited to
the following action:
1.  Ensure that all employees who work in an aerial lift are provided with
and use a safety
harness and lanyard which is correctly donned and used according to the
manufacturer's
recommendations.
