Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer failed to provide a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees: a. On or about June 1, 1999, at the jobsite located at 4201 South King Drive in Chicago, Illinois, employees working from an aerial lift were exposed to injuries from falls due to the lack of (personal) fall protection systems required by the American National Standards Institute's standard for boom-supported elevated work platforms.
