North Carolina General Statue 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to:: a)struck-by and/or crushed by vehicular traffic - an independent truck driver was seriously injured when he was struck by an Ottawa Commando, unit #655053, tractor. The active pedestrian and/or conveyance entrances to the facility transportation and dispatch offices were not posted with signs indicating pedestrian traffic. One feasible and acceptable abatement method, among others, to correct this hazard is to post signs indicating pedestrian traffic. The active pedestrian and/or conveyance entrances to the facility transportation and dispatch offices were inadequately illuminated. One feasible and acceptable abatement method, among others, to correct this hazard is to re-design the facility exterior lighting installations.
