North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a)falling to lower elevation - employees were allowed to
use fourteen hoists to lift, support, and transport
employees who worked off seven mobile work platforms
"catwalks":
One feasible and acceptable abatement method, among others, to
correct this hazard is to not allow employees to be lifted,
supported, or transported by hoists.

North Carolina General Statue 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees  conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a)struck-by and/or crush-by - employees were allowed to
stand and reach underneath hoisted loads, i.e., sub-
floors, walls, ceilings, manufactured homes chassis and
axles, gantry nailers, and work platforms "catwalks":
One feasible and acceptable abatement method, among others, to
correct this hazard is to not allow employees to work under or
reach underneath hoisted loads.

employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a)falling to lower elevation - employees were allowed to
use a Manhandler, model 2031, scissor-lift, that was
missing its guardrail at its opening.
One feasible and acceptable abatement method, among others, to
correct this hazard is to equipped the scissor-lift with
guardrails or equivalent.
