North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)falling to lower elevation - employees were allowed to use fourteen hoists to lift, support, and transport employees who worked off seven mobile work platforms "catwalks": One feasible and acceptable abatement method, among others, to correct this hazard is to not allow employees to be lifted, supported, or transported by hoists. (Reference: American National Standards Institute, for Overhead Hoists (Under hung), ASME B30.16-1993, Chapter 16-3, Section 16-3.1.3(j); CM Lodestar, Operating, Maintenance, & Parts Manual, Safety Precautions Warning #31).

North Carolina General Statue 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)struck-by and/or crush-by - employees were allowed to stand and reach underneath hoisted loads, i.e., sub- floors, walls, ceilings, manufactured homes chassis and axles, gantry nailers, and work platforms "catwalks": One feasible and acceptable abatement method, among others, to correct this hazard is to not allow employees to work under or reach underneath hoisted loads. North Carolina General Statue 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)falling to lower elevation - employees were allowed to use a Manhandler, model 2031, scissor-lift, that was missing its guardrail at its opening. One feasible and acceptable abatement method, among others, to correct this hazard is to equipped the scissor-lift with guardrails or equivalent. (Reference: American National Standards Institute, for Elevating Work Platforms, ASME A92.3-1973, Section 5.2)
