North Carolina General Statue 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees  conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees, in that employees were exposed to a crushing hazard:
a)slasher room -- employees were using a Coffing 1 ton
electric overhead chain hoist mounted on steel I-beams.
The hoist was used to lift 1000 lb. warper beams of
cloth. Dated hoist inspection reports and records were
not maintained.(Ref. ANSI B30.16-1981 Sec. 16-2.1.5(a)).
One feasible method of abatement, among others, is to
perform and document annual and frequent inspections on
the crane.tions
