Carolina:  The
employer did not furnish employment and a place of employment which were
free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to the potential structural
failure of the
equipment:
a)Southeast Corner of Floor, Plant 1 - hoisting unit fabricated in house by
maintenance staff consisting of a welded framework on wheels with an
attached 1000 lb. capacity Dayton Winch, Mod 5W659 used for hoisting and
relocating 400 lb. final assembly board.  Hoisting unit had not been
operationally tested or load tested prior to it's initial use.
One feasible and acceptable abatement method, among others, to correct
this hazard is to
have design evaluated by a professional engineer and conduct an
operational test in
accordance with ANSIB30.16-1981 16-1.3.2 and 16-2.2.1 (b) and 16-2.2.2 (b).
