Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the inhalation hazards associated with exposure to an uncontrolled release of Gallium Arsenide fume. a) Crystal Pulling Room The "O - ring" seal between the upper assembly and lower chamber of Crystal Puller #11 failed at 0500 hours on August 3, 1998 due to stripped flange bolts, resulting in employee exposure to an uncontrolled release of Gallium Arsenide fume. Violation observed on or about 8/6/98. Abatement Notes: Among others, one feasible and acceptable abatement method to correct this hazardous condition is: 1)Develop and implement a maintenance program for the inspection, maintenance, and replacement of bolts in conformance with the requirements set forth in Chemical Manufacturing Association, Mechanical Integrity, Sections 4.4 and 5.0, September 1994. ABATEMENT NOTE: This violation requires certification and documentation as appropriate proof of abatement.e
