North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a)jobsite, employees erected and used wooden job-made
ladder that presented hazards to employees.  Side rail
lumber was not stamped with lumber grade; cleats were not
attached with 3 staggered nails; no filler block placed
between cleats; ladder was not positively fastened to
secure object at top point of bearing; a safe landing was
not provided at top of ladder and between multiple
ladders.
