and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a)facility; staging - the monorail system has not been load
tested.
Reference:  ANSI 30.11-1980; paragraph 11-2.2.2 (a)
"Prior to initial use, all new, extensively repaired, and
altered equipment should be tested and inspected by, or
under the direction of, an appointed or authorized
person, and a written report should be furnished by such
person..."
