Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Employee(s) dumping paper and other debris from the trash boxes into the
baler
compactor were exposed to falls, up to six-feet, when climbing on the baler
compactor to reach the trash box, and while standing inside the trash box,
which
is elevated about six-feet above the concrete floor, unsecured, on the two
forks
of an industrial lift truck.  Employee(s) enter the elevated trash box to
help dump
its contents into the baler compactor.
Among other methods, one feasible methods to correct this hazard include,
but
are not limited to the following: Prohibit the use of lift trucks for
elevating
personnel, unless employees are authorized by a competent person, and then
only
with the use of a safety platform that meets the design, construction, and
operational requirements of the current (1993) revision of the American
National
Standard, B56.1.
No abatement certification is required for this item.
