ORS654.010: The employer did not furnish employment which was safe and healthful for employees, and did not adopt and use such practices, means, methods, operations and processes as are reasonably necessary to render such employment and place of employment safe and healthful: (a) The employer did not inspect, cause inspection of or require documentation of inspection of all worksites where workers were assigned to determine potential safety and health hazards of those assignments. (b) A job description and/or job safety analysis was not made to determine the hazards inherent in the jobs to be performed by workers in a high hazard industry. (c) An evaluation of a client's workplace safety and health program, previous injury history and/or training and supervisory provisions for non-english speaking workers was not conducted. (d) Means or methods were not developed and implemented to effectively train workers unfamiliar with machinery on the control of hazardous energy including definition(s) of terminology used in the firm's Lock-out Procedure such as "personal lock," "power source," "switch or valve," "energy is flowing" and "energy under control".
