falling
under moving rail cars and being crushed between rail cars due to damaged
equipment such as fixed ladders, grab irons, foot sills, riding platforms,
and
cutting levers:  1) At the Bloom Caster - Caster Flat 9004 had a damaged
grab
iron.  2) At the Bloom Caster - Caster Flat 9029 had a damaged grab iron.
3)
At the Bloom Caster - Scrap Car 4177 had a damaged foot sill and grab
iron.  4)
At the Bloom Caster - Scrap Car 4013 had a damaged foot sill.  5) At the
Flint
Ridge Yard - Caster Flat 9005 had damaged foot sill, grab iron, and hand
brake
wheel.  6) At the Flint Ridge Yard - Caster Flat 9020 had a damaged grab
iron.

Among other methods, one feasible and acceptable abatement method would
include, but would not be limited to:  establish and implement a periodic
maintenance program and insure that damaged rail car appliances are
identified
and repaired.d
