General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Employee(s) working in the "D" Cargo Pit of a DC-8 Aircraft were exposed to struck by hazards when the barrier door was not properly secured. The partition was secured with safety wire instead of the fastener that it was designed to accept. This condition created a hazard which when side pressure was applied to the barrier door causing the safety wire to break, disengaging the top portion of the barrier door. On May 22, 2000, an employee working on the opposite side of the barrier door was struck in the head when the safety wire securing the top corners broke. Among other methods, one feasible and acceptable method of abatement is to use the fasteners, or latches, recommended by the manufacturer.
