Fall hazard or being struck by defective elevator due to lack of required
semi and annual
inspections and tests on the special purpose elevator.
Among other methods of abatement:  The conducting of tests and inspections
in accordance with
the American Society of Mechanical Engineers standard A 17.1.sections 1001
and 1002.

The hazards of employees being struck-by elevator car as it moves between
levels in  the shaft
or workers falling down the elevator shaft due to the lack of guarding in
front of the special
purpose elevator openings.
Among other abatement methods one feasible and acceptable method is to
provide and require
the use of guarding or enclosures at all elevator landings of the special
purpose elevator.
