In the hydrostatic test area a gasket on a heat exchanger which was being
hydrostatically tested
was not fastened to the flange in such a manner as to prevent the gasket
from being destroyed
while undergoing hydrostatic testing.
Feasible methods to correct the hazard include, but are not limited to,
ensuring bolts are
properly torqued and tightened in the appropriate sequence or using a
gasket material designed
to withstand the pressure at which the item is being tested.
