SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,
Code of
Laws, State of South Carolina, 1976, as amended, as follows:
A.  On or about August 28, 1999, employer knew or should have known that
employees working around reactor #3 flash tank were exposed to the hazards
of
fire, explosion, or release of toxic vapors by using a black rubber type
gasket rated at a maximum temperature of 250 degrees fahrenheit to seal the
manway opening on reactor #3 flash tank which operates at 52 PSIG and 700
degrees fahrenheit.  A useful and feasible means of abatement, among
others,
is to use a gasket that is rated for the type of operation and operating
temperature of the system in which it is used at the Shakespeare
Plant.peare
