SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards
which may cause death or serious physical harm to his employees and comply
with this
regulation and other occupational safety and health rules and regulations
promulgated under
Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as
amended, as
follows:
A. Employer knew or should have known that workers in the pipelaying and
invert crew
entering sewer manholes such as the one adjacent to Highway 17 were
exposed to the
hazards associated with entry into confined spaces.  A feasible and useful
means of
abatement, among others, is:
1)Test the sewer manholes for oxygen level, flammability, and known or
suspected toxic substances such as but not limited to sewer gases and
carbon
monoxide, prior to entry.
2)Provide adequate ventilation at the site for sewer manholes when needed.
3)Provide rescue equipment at the site such as, but not limited to:
tripod, safety
line, harness, and proper respiratory equipment.
4)Provide a written confined space program at the jobsite.
