A. Employer knew or should have known that a Lull Highlander II forklift,

number F3, operated in reverse gear with obstructed view to the rear on
site with an
inoperable reverse signal alarm and without use of an observer constituted
a hazard of an
employee being backed over.  A feasible and useful method, among other
methods,
to
correct this hazard would be to have an operating reverse signal alarm on
the forklift.
