SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,

A.  Employer knew or should have known that workers in the pipelaying, crew
entering live sewer manhole at east end of site were exposed to the hazards
associated with entry into confined spaces.  A feasible and useful means of
abatement, among other methods is:  1) Provide a written confined space
program.  2) Test the manhole for oxygen level, flammability, and known or
suspected toxic substances such as, but not limited to sewer gases and
carbon
monoxide prior to entry.  3) Provide adequate ventilation at the site for
manhole when needed.  4) Provide rescue equipment at the site such as, but
not
limited to a tripod, safety line, harness and proper respiratory equipment.
