A. Employer knew or should have known that operators near isocyanate tank

exposed to hazard of eruption of Rubinate 7302 from the tank.  A feasible
and
useful method to correct this hazard, among other methods, is to include
changing of maintenance procedures to include complete system inspection
before, during and after each tank filling, with logging of tank levels and
pressures; and locate pressure relieving devices so nature of the tanks
contents does not hinder their functioning.
