A.  Employer knew or should have know that employees working on the W019
truck
were exposed to the hazards of being hit or run over by truck if it engages
into gear while accelerator is depressed by bar between seat and
accelerator
of vehicle while operating the hydraulic system on the outside of the cab.
A
feasible and useful method to correct the hazard, among other methods, is
to
repair the levers in the cab of the truck that will enable the hydraulics
to
function without having to depress accelerator with metal bar and by
discontinuing the use of the bar.

obstructed view to the rear that had no audible backup alarm or observer
when
it was operated in reverse gear, which exposed mechanics and other
equipment
operators to the hazard of being backed over and crushed.  A feasible and
useful means of correcting the hazard, among other methods, is to repair
present backup alarm to audible levels or to install audible backup alarms.
B.  Employer knew or should have known that the L020. Kawasaki loader,
with an
obstructed view to the rear had no audible backup alarm or observer when it
was operated in reverse gear which exposed mechanics and other equipment
operators to the hazard of being backed over and crushed.  A feasible and
useful means of correcting the hazard, among other methods, is to repair
present backup alarms to audible levels or to install audible backup
alarms.
