A. Employer knew or should have known employee working on elevated work
platform not
secured to carriage or forks of front end loader in excess of 19 feet high
at the front side of
building was exposed to the hazard of employee falling due to platform
slipping.  A feasible
and useful means of abatement among others is to ensure work platform is
secured to
carriage or forks of front end loader.e
