SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards
which may cause death or serious physical harm to his employees and comply
with this
regulation and other occupational safety and health rules and regulations
promulgated under
Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as
amended, as
follows:
A. Employer knew or should have known that employees erecting 14 metal
plate connected
wood trusses at peak of roof in excess of 22 feet high were exposed to the
hazard of the
trusses toppling and falling to the floor below due to only 25 1/2 inch
long spacer blocks
along with 3 end braces being used as temporary bracing.  A feasible and
useful method to
correct this hazard, among other methods, is to follow the instructions
and recommendations
in the HIB-91 Summary Sheet-Commentary and Recommendations for Handling,
Installing,
and Bracing Metal Plate Connected Wood Trusses or equivalent and use
bracing such as but
not limited to lateral bracing, diagonal bracing, and cross bracing.
