SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards
which may cause death or serious physical harm to his employees and comply
with this
regulation and other occupational safety and health rules and regulations
promulgated under
Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as
amended,
as follows:
A.  Employer knew or should have known that employees working around the
melting
furnace number one using an overhead crane to exchange charging electrodes
from the
furnace in the melt shop was exposed to the hazard of being burn or struck
by molting metal
from the explosion of the intermixing of water and melting metal in the
furnace due to
overhead line breakage.  A feasible and useful means of abatement, among
others, is to
establish written procedures to include:  training employees to deal with
water lines and pipe
fittings breakage on furnace roof; safe emergency shut down procedures
during water line
and pipe fittings breakage; install protection barriers around water lines
and pipe fittings to
avoid contact with electrodes or install a pressure sensor device to
automatically shut off
water in case of water lines or pipe fittings breakage on furnace roof.
