Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of a defective weld repair made to a crack on
the bridge girder of
the C3 underhung crane.
a)A crack on the bridge girder of the C3 underhung crane was identified by
the crane
inspections conducted at the plant.  On or about November 28, 1998 a weld
was made
to the crack, but the weld was not evaluated for its weight carrying
ability or structural
integrity before the C3 crane was put back into service.  The C3 crane was
operated for
two days until an inspection by qualified personnel revealed that the
attempted repair had
been defective.
Among others, one feasible and acceptable method to abate this hazard is
to
have
structural repairs and crane modifications checked by a qualified engineer
or equipment
manufacturer prior to placing the equipment back into service.
ANSI Standard B30.11, SAFETY STANDARDS FOR CRANES, DERRICKS, HOISTS,
HOOKS, JACKS, AND SLINGS (MONORAIL SYSTEMS AND UNDERHUNG
CRANES).
Abatement documentation is required for this item in accordance with the
requirements of
29 CFR 1903.19(d).s

were exposed to the hazard of physical deformity of the crane, negative
camber on south bridge
girder of the C3 underhung crane:
a)  The south bridge of the C3 underhung crane was identified by the crane
inspection(s)
conducted at the plant to have a negative camber.  On or about November
18, 1998 the
condition was identified during this inspection.  The crane continued to
be operated until
on or about November 23, 1998 when a crane inspection by qualified
personnel
confirmed the condition and revealed that continued use could result in
damage to crane
and/or loss of the load.
Among other methods, one feasible and acceptable method of abatement to
correct this
hazard is to replace the bridge girder.
