employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees; specifically:
The Radial Saw used to cutoff metal bars, angles, tubing, etc. extended
beyond the edge
of the table and beyond the position needed to complete the cut in
repetitive (production)
operations; and the cutting head did not return gently to the starting
position when
released by the operator.

likely to cause death or serious injury to employees, specifically:
The personnel platform located in Bays 6 and 7 and used on a forklift
truck
for
removing, installing, or working on trailer doors:
a)Did not have a guard rail of not less than 36 in. or more than 42 inches,
nor was a midrail installed between the top railing and the platform on all
sides; and
b)Lacked toeboards, minimally 4 inches high; and
c)Was not securely attached to the lifting carriage or forks.
