conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees; specifically:
The employer failed to require and enforce the usage of seatbelts on the
Mitsubishi
forklift truck being used in the yard and the warehouse.  The operators
are exposed to
a ramp with a height of 51 inches, which would allow the forklift truck to
tip or roll
over, causing serious injury or death to the operator.
