AT&T TELECOMMUNICATION TOWER SITE, 6788 HIGHMILL STREET, JACKSON
TOWNSHIP, OHIO:
Tuesday, November 9, 1999, during the construction of the two hundred and
fifteen-foot, FWT,
self-supporting telecommunication tower, employees were exposed to the
hazard of falling,
getting caught by and/or striking the tower while being transported by the
crane.
a.An employee, who was detaching a sling from the top of a tower leg, was
lifted to and
lowered from the work area by the crane.  The employee was attached to the
crane hook
using a body belt and lanyard.
b.An employee, who was preparing to descend from the tower, was lowered to
the ground
by the crane. The employee was attached to the crane hook using a body
belt and
lanyard.
Among other methods, feasible and acceptable methods to correct the
hazardous condition may
include:
1.Utilize an approved personnel platform with the use of a crane in
accordance with 29
CFR 1926.550(g).  Assure only trained and authorized personnel are
permitted to operate
the crane, conduct pre-lift inspections, meetings, trial lifts to assure
all systems, controls
and safety devices are activated and functioning properly.
2.Utilize an approved safety climbing device and fall arrest system while
climbing the
tower.
