Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to decomposition and explosion hazards from organic peroxides in the work area. On 12/1/99 and 12/13/99 at least 4-5 gallon containers of Dicumyl Peroxide, 4-5 gallon containers of 2,5-Dimethyl-2,5-Di(t-butylperoxy)Hexane and 2-5 gallon containers of DiCumyl(K-9607) peroxide were in the compounding area. The specific deficiencies noted on the site include but are not limited to: 1. Excessive amounts of organic peroxide in the processing area. The amount should be limited to the quantities needed for immediate use. 2. Storage of peroxide in the process area. Storage must be in a properly constructed and equipped storage room or cabinet. 3. Storage of a Class III peroxide with other chemicals on the storage racks. Storage must be separate. Among other methods, one feasible and acceptable method to correct this hazard is to follow the provisions of the Ohio Fire Code 1301:7-7-37(Chapter 37), effective March 30, 1998; NFPA 432,1997; and Factory Mutual Property Loss Prevention Data Sheets, March 1972. This includes storage in separate rooms or cabinets.
