Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential crushing hazard by entering into the crushing chamber of the Cedarapids jaw crusher to dislodge a piece of oversized material without locking and tagging out the crusher prior to entry: a.Job site - On 5/8/01, employees were exposed to a potential crushing hazard, which could result in extreme injury or death, when they entered the crushing chamber to remove a jam of oversized material, and lockout/tagout procedures were not followed to prevent the crusher from being inadvertently activated while the employee was in the chamber. Among other methods, one feasible and acceptable means of abatement would be to disconnect all energy sources, and apply lock(s), where each authorized employee places his/her lock(s)so that all energy sources are positively isolated.
