Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing due to being struck by a moving elevator car;
and/or having parts of
their bodies being caught in the sheer points created by the elevators
moving between floors due
to inadequate elevator hoistway doors/gates and/or lack of elevator car
gates;
and/or employees
falling down the elevator shaft due to a lack of parking devices on the
hoistway gates allowing
the gate to be openened when the elevator car is not in the landing zone;
and/or employees were
unable to call for help should a problem occur due to lack of an alarm
system; and employees
had an increase chance of accident due to an inadequate number of lighting
sources:
On 12/21/98
(a)Elevator #1 - First floor hoistway landing opening gate, which was 45
1/2" high,
did not meet the  6' minimum height requirement in accordance with American
National Standards Institute (ANSI) A17.3, Safety Code for Existing
Elevators
and Escalators, paragraph 2.6.1(b).
(b)Elevator #1 - Second floor hoistway landing opening gate, which was 52"
high,
did not meet the 6' minimum height requirement in accordance with ANSI
A17.3,
paragraph 2.6.1(b).
(c)Elevator #1 - Third floor hoistway landing opening gate, which was 52"
high, did
not meet the 6'minimum height requirement in accordance with ANSI A17.3,
paragraph 2.6.1(b).
(d)Elevator #3 - Elevator car did not have a front or rear gate in
accordance with
ANSI A17.3, paragraph 3.4.2.
(e)Elevator #1 - Hoistway landing opening gate did not have a parking
device to
keep the hoistway gate from opening or unlocking when the elevator car was
outside the landing zone in accordance with ANSI A17.3, paragraph 2.7.2.
(f)Elevators #1,# 2, and #3:  Elevators were not equipped with emergency
signaling
devices (alarms) in accordance with ANSI A17.3, paragraph 3.11.1.
(g)Elevators #1, 2, 3:  Elevators were not equipped with a minimum of two
lamps
in accordance with ANSI A17.3, paragraph 3.4.5.
One feasible method of abatement is to install adequate hoistway landing
gates where required,
install elevator car gates where required, install parking devices where
required, install an
emergency signaling system in all elevators, and install a second lamp in
each elevator.

were exposed to the hazard of being pulled into and crushed by the
rotating parts of a pipe
threader machine:
Week of 12/7/98
(a)Maintenance Shop - A Rigid pipe threader machine, which was operated by
a
toggle switch that would not automatically shut the machine off if its
operator was
inadvertently caught up in its rotating parts.
Abatement Notes:  Among other methods, one feasible method to reduce this
hazard is to
establish and enforce adequate safe operating procedures such as those
stipulated in American
National Standard for Portable Pipe Threading Machines (ANSI B208.1-1982).
Elements
essential to an acceptable safe operating procedure include at a minimum:
1.Installation of a momentary contact device (deadman foot switch) to
control
power input to the machine.  This device would automatically stop the
machine
and the rotating pipe if the operator's foot was removed from the foot
switch for
any reason.
2.Provide training and education to the machine operators concerning all
the
appropriate procedures for the safe operation and use of this type of
machinery.
