(a)On or about December 6, 1999, a 40 gallon drum of ammonia solution and

gallon drum of bleach (sodium hypochlorite) were stored next to each
other.  The
drums were used to dispense the products for cleaning.
Among other methods, one feasible and acceptable abatement method is to
store the
drums in separate rooms.

maintenance and access to valves:
(a)Maintenance employees were required to perform maintenance on the
ammonia
refrigeration system without ammonia monitoring or adequate safety
procedures.  One
task involved the draining of oil from a collection vessel.  The end point
of the task
was determine when ammonia began to escape from the drain valve.
Among other methods, one feasible and acceptable abatement method is to
follow the
guidelines set forth by the American National Standards Institute/American
Society of
Heating Refrigeration and Air-Conditioning Engineers publication
ANSI/ASHRAE 15
"Safety Code for Mechanical Refrigeration" or by the guidelines set forth
by the
International Institute of Ammonia Refrigeration publication IIAR Bulletin
No. 110
"Guidelines for: Start-up, Inspection and Maintenance of Ammonia Mechanical
Refrigerating Systems."
