Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard: (a)On 28 JUL 00, and at times prior thereto, millwrights who were required to use a CM series 632 Trolley and overhead hoist for replacement of the esco valve were exposed to the crushing hazard of an improperly assembled and installed trolley and hoist. Among other methods, one feasible and acceptable abatement method to correct this hazard is to train any employee who is authorized to use a trolley how to assemble the trolley and how to install it for use in accordance with the instructions and: 1. Train employees how to estimate load and trolley and hoist sizes required for specific jobs. 2. Familiarize employees with parts and part names of trolleys. 3. Train employees how to assemble trolleys including properly mounting the side frames on the load bars. 4. Train employees how to adjust the collar and load bar to properly fit beam flange width according to the manufacturer's instructions and the chart located thereon. 5. Train and require employees to measure installed trolley's distance between trackwheel flanges. Note: Abatement certification and supporting documentation are required for this item.
