section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were likely to cause
death or serious physical harm to employees in that employees were
exposed to being struck by a falling custom made lifting device:
Dura Bar used a custom made lifting device to lift scraps and other
metal bars weighing approximately 6,000-27,000 pounds.
Feasible and acceptable abatement methods include, but are not
necessarily limited to:
Comply with ANSI B30.20 - Below the Hook Lifting Devices and:
1.Provide the rated load on any custom made lifting device.
2.Design the lifting device to withstand the forces imposed
by its rated load with a minimum design factor of 3.
3.Provide initial, frequent, and periodic inspections of
the lifting devices.
4.Ensure that initial proof testing of 125of the rated
load was conducted.
