section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to being struck by a falling custom made lifting device: Dura Bar used a custom made lifting device to lift scraps and other metal bars weighing approximately 6,000-27,000 pounds. Feasible and acceptable abatement methods include, but are not necessarily limited to: Comply with ANSI B30.20 - Below the Hook Lifting Devices and: 1.Provide the rated load on any custom made lifting device. 2.Design the lifting device to withstand the forces imposed by its rated load with a minimum design factor of 3. 3.Provide initial, frequent, and periodic inspections of the lifting devices. 4.Ensure that initial proof testing of 125of the rated load was conducted. CERTIFICATION REQUIRED FOR THIS ITEM
