OOOOOOOOVSection 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were subject to a fall from a platform lifted by a rough terrain forklift. North Ave/approximately 13 feet above grade - An employee preparing to install windows from a forklift elevated personnel platform was exposed to the following hazards: A. The maximum workload was not indicated on the platform. B. The platform was attached to the forklift by a knotted rope through wafer board. C. Restraining means for securing personnel was not provided. D. Access/egressing the platform from a window opening on the second floor. Among feasible means of abatement would be to comply with the American Society of Mechanical Engineers, ASME B56.6-1992 Safety Standard for Rough Terrain Forklifts
