Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to a fall hazard, on 9/4/99.
Hydro Unit:
(a)During an unplanned power failure, there was no source of backup
electric
power in order to position the dryers, to lower the dryer access ramps,
and to
run the exhaust fans;
(b)The ventilation system for the control room was not maintained
adequately to
ensure the integrity of the atmosphere inside the control room during a
process
upset condition.

employees on 09/04/99:
Hydro Unit:
(a)The system for introducing water into the dryer from a fire hose
through the
trunnion of the dryer was not designed adequately to control and safely
dispose
of the contents of the dryer in the event of a possible decomposition;
and,
(b)The thermometers for the dryers were not calibrated as often as stated
in the
written procedures for the Hydro Unit.
Hydro Unit: For the A/C system and the purafil system.
