support material being lowered to the ground. On 7/11/00, an employee was
exposed to a fall hazard
in that the common anchorage point for both the lifeline and the material
handling failed, catapulting
him approximately 60 feet to the ground.
ABATEMENT NOTE: AMONG OTHER METHODS, ONE FEASIBLE MEANS OF
ABATEMENT TO CORRECT THIS HAZARD IS TO TIE THE LOWERING LINE AND THE
CLIMBING LINES INTO DIFFERENT LIMBS.
