North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish
each of his employees conditions of employment and a place of
employment free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:  hoist failure that would subject
employees to falling overhead material hazards.

Systems 1 ton electric powered chain hoists to lift 200
Lb. rolls of paper used in a laminating process. Periodic
inspections of the hoists were not performed.(Ref ANSI
B30.16-1981, Sec. 16-2.1.3)
One feasible and acceptable method, among others, to
correct the hazard is to conduct the inspections in
accordance with ANSI B30.16-1981, Sec. 16-2.1.3, Tables
1 and 2.
