and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a) Roberts Street facility  --  the Ingersoll Rand monorail
crane was not load tested prior to initial use.

NOTE: Among other methods, one feasible and
acceptable method to correct this hazard is to
load test all cranes as per ANSI B30.11-1980.
b) Roberts Street facility  --  no load test report was
available for the Ingersoll Rand monorail crane outside
the building.
Reference:  ANSI B30.11-1980: paragraph 11-2.2.2(a)
Test reports shall be placed on file.
NOTE: Among other methods, one feasible and
acceptable method to correct this hazard is to
conduct the load test and have the test report
readily available.
c)  Roberts Street facility  --  complete inspection of the
Ingersoll Rand monorail crane outside the building was
not performed at intervals as defined in ANSI B30.11-
2.1.1(b)(2) with inspection records maintained.
Reference:  ANSI B30.11-1980; paragraph 11-2.1l3(b)
complete inspection of the crane or
monorail system shall be performed at
intervals as defined in 11-2.1.1(b)(2).
NOTE: Among other methods, one feasible and
acceptable method to correct this hazard is to
conduct periodic inspections of the crane and
maintain records of the inspections.
