North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a) facility; new plating room/Budgit hoist - monorail system
used by employees has not been load tested
b) facility; plating room/Coffing hoist - monorail system
used by employees has not been load tested
c) facility; plating room/Dayton hoist - monorail system
used by employees has not been load tested
d) facility; plating room/Lodestar hoist - monorail system
used by employees has not been load tested
e) facility; plating room/CM hoist - monorail system used by
employees has not been load tested
Reference:  ANSI B30.11-1980 Section 11-2.2.2  Prior to
initial use all new, extensively repaired and altered
equipment should be tested and load test results shall be
placed on file.
Note:  Among other methods, one feasible and acceptable
abatement method to correct this hazard is to conduct a load
test on all cranes in the facility.

repaired cranes and monorail systems shall be inspected...as
per paragraph (b) on a frequent and periodic basis.
Note:  Among other methods, one feasible and acceptable
abatement method to correct this hazard is to conduct
inspections and continue them periodically and frequently.y
