the building
at the 6J7 line disconnects.
On or about 9/21/99, during the switching procedures, employees were
exposed to an
intense high voltage 4KV flash due to the use of a defective solid blade
disconnect, while
attempting to energize the 6J7 three phase regulator.
AMONG OTHER METHODS ONE FEASIBLE AND ACCEPTABLE MEANS OF
ABATEMENT WOULD BE TO REPAIR OR REPLACE THE SOLID BLADE
DISCONNECT.
