Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a high voltage electrical flash associated with a defective solid blade disconnect. a. Location - Olneyville Substation on Dike Street, Providence, RI, inside the building at the 6J7 line disconnects. On or about 9/21/99, during the switching procedures, employees were exposed to an intense high voltage 4KV flash due to the use of a defective solid blade disconnect, while attempting to energize the 6J7 three phase regulator. AMONG OTHER METHODS ONE FEASIBLE AND ACCEPTABLE MEANS OF ABATEMENT WOULD BE TO REPAIR OR REPLACE THE SOLID BLADE DISCONNECT.
