recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of amputation while working with the Muller
Martini machine that
could not be reliably shut off quickly in an emergency:
a) Bindery Department:  On or about 03/22/00, the stop button on the
Muller Martini
machine was not operational exposing employees to the hazard of being
caught up in the
equipment and not able to shut it off.  AMONG OTHER METHODS ONE FEASIBLE
AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO
ensure that emergency stop devices are in working order.  To perform
routine inspections
and corrective maintenance measures to insure that all guards and safety
features are
retained and function properly.
