Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of working under and/or around elevated
vehicles that were not
supported in such a way as to provide sufficient stability to ensure that
the vehicle would not
fall.  The supports did not meet the requirements of ASME PALD standards.
a)  On or about May 7, 1999, an employee of Sharp Parts Auto Recyclers,
Inc., was
fatality injures when the vehicle he was working under fell on him.  The
vehicle was
improperly supported by using a two wheel rim with a two by four placed
across each
rim.  One rim placed was under vehicle's frame at approximately the mid
point of the
driver's door and the other at approximately the mid point of the
passenger door.  There
were no measures taken to prevent the car from rolling backwards.
Among other methods, one feasible and acceptable method to abate this
hazard is to purchase
and use commercially manufactured vehicle support stands that incorporate
the requirements of
ASME PALD.
Abatement documentation for this item is required in accordance with the
requirement of
29 CFR 1903.19(d).

that did not meet ASME PALD Standards.
a)  Fabricated vehicle support stands did not have their rated
capacity identified as required in the General Requirements
for all Portable Automotive Lifting Devices Section 1
Paragraph 1.1, and were not proof load tested to 150of the
rated capacity as required in Part 4 paragraph 2.7 of the
standard.
Among other methods, one feasible and acceptable method to abate
this hazard is to purchase and use commercially manufactured
vehicle support stands the incorporate the requirements of ASME
PALD.
Abatement documentation is required for this item in accordance
with the requirements of 29 CFR 1903.19(d).s

not configured such as to aid in the proper positioning,
supporting, and retaining of the load in that the resting
surface was irregular, smooth, and did not provide an adequate
contact surface between the vehicle and the saddle surface.

this hazard is to purchase and use commercially manufactured
vehicle support stands that incorporate the requirements of ASME
PALD.

employees were exposed to the hazards of fire due to unsecured
acetylene cylinders falling over and damaging or breaking off the
regulator causing uncontrolled release of acetylene.
a)  In the yard there was an unsecured acetylene cylinder
being used for oxygen/acetylene cutting.  This cylinder was
free standing on a dirt surface.
Among other methods, one feasible and acceptable method to abate
this hazard is the use of a two cylinder hand cart/truck designed
to hold and transport oxygen/acetylene cylinders while in use.

with the requirements of 29 CFR 1903.19(d).s
