Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to The HAZARD OF BEING STRUCK-BY THE LOAD: a)Bekaert machines Nos. 1 & 2, on or about 9/3/99: Employees were not checking the upper limit device for the C.M. Model Loadstar electric chain hoist, in that the hoist chain was kinked so as not to allow limit device to be functionally checked. Hoist is used by employees to place empty & remove full reels of wire from the Bekaert machines. ABATEMENT NOTE: Ensure that upper limit devices are checked at the beginning of each shift, per ASME/ANSI B30.16-1987, Section 16-2.1.2(b), American National Standard for Overhead Hoists. b)Maintenance Shop, on or about 9/9/99: Yale one-ton jib crane hook throat latch was inoperative. Employees use jib crane for lifting various items. c)F Syncro Rod Raw machine, coiler end, on or about 9/2/99: Hook on machine hoist used to lift & lower elephant trunk had the throat latch missing. ABATEMENT NOTE: Ensure that hook latches are checked for proper operation per ASME/ANSI B30.16-1987, Section 16-2.1.2(e), American National Standard for Overhead Hoists. d)F Syncro Rod Raw machine, coiler end, on or about 9/2/99: Pendant control for machine hoist controller functions was not clearly labeled as to indicate the direction of resultant motion. ABATEMENT NOTE: Ensure that each control actuator is marked to indicate direction of resultant motion per ASME/ANSI B30.16-1987, Section 16-1.1.2, American National Standard for Overhead Hoists. e)Bekaert machines Nos. 1 & 2, on or about 9/3/99: Below-the-hook lifting device used with the C.M. Model Loadstar 1/2-ton electric chain hoist SN. L246DQ did not have its rated load marked on it. f)Bekaert machines Nos. 3-7, on or about 9/3/99: Below-the-hook lifting device used with the Harrington 1/2-ton electric chain hoist did not have its rated load marked on it. g)Bunching area, on or about 9/8/99: Below-the-hook lifting device used with Coffing model EC-2016-3 1/2-ton electric chain hoist SN. EC1A3252U did not have its rated load marked on it. ABATEMENT NOTES: 1.Ensure that all below-the-hook lifting devices are load rate tested per American Society of Mechanical Engineers, ASME B30.20-1993, Section 20-1.4.2(a) & (b). 2.Ensure that all below-the-hook lifting devices are marked with their rated load & identification per American Society of Mechanical Engineers, ASME B30.20-1993, Section 20-1.2.1(a) & (b). 3.Ensure that all below-the-hook lifting devices are inspected per American Society of Mechanical Engineers, ASME B30.20-1993, Section 20-1.3.1(a) & (b). h)High Speed Bunching Area, on or about 9/8/99, Bridge crane: No stops were provided on trolley rail for Coffing model EC-2016-3, 1/2-ton electric chain hoist SN. EC1A3252U. i)High Speed Bunching Area, on or about 9/8/99, Bridge crane: One stop was missing on south end of crane runway. ABATEMENT NOTES: 1.Ensure that stops are provided at the limits of travel of the trolley per American National Standard ANSI/ASME B30.17, Section 17-1.8.1(a), (b) & (c). 2.Ensure that stops are provided at the limits of travel of the bridge per American National Standard ANSI/ASME B30.17, Section 17-1.3.2(b).
